Instead of maximizing a Quicktime window, I had iPhoto up so I made that full-screen, then exited. Minimize button was back! It seems to do that randomly with any app you make full-size. I followed the directions for Quicktime and then tried it with iPhoto. This is a really annoying problem! I just had this issue with Chrome and after some searching, ended up at this blog post.
